Had this same problem with my 636. Ended up being a little tiny crack in my radiator. Not enough to leak coolant (dripped once and a while) but enough to let some pressure out, don't ask me how it worked lol. Could be why your cap didn't fly off when you took it off due to pressure being lost through the crack. All I did was find an aluminum welder, had him patch the crack with a little bead action, then flushed the system and was golden. Bike only gets to about 215 after a good long session.
Not saying this is a culprit but check for any cracks or pressure leaks to be sure.
