Always carry with one in the chamber on my 1911 and the hammer back. .380 is the same way. the holsters I have don't allow access to the trigger until drawn. No reason not to carry in a ready-to-fire state unless by some crazy circumstance your state doesn't allow it.

my step dad is a 28 year state trooper, he was showing me this EXACT situation one day, he wears a heavy belt for just this, he can pull and chamber a round by A thrusting the slide against his boot, or B against his belt

depends on what kind of sights your gun has. Mine has rounded sights so it's LESS likely to get snagged or caught on clothing and things. Though I can do this with my H&K USP. It's got the older, square sights.

If you feel the need to carry, then it should be always ready to go. Always.
The theoretical 21 feet can be closed real quickly by an attacker.

Chambered, no question.

I usually carry my Xd .40 with one chambered with my Walther .380 in the glovebox.
